Life on Track Operational Manual

The Life on Track Operational Manual clearly describes the Life on Track process. It also provides other important information such as:

  • details of important documents used in service delivery
  • information about service staff and their roles and responsibilities
  • lessons from other court based programs
  • stakeholder relationships and information
  • the Life on Track Levels of Service Guidelines.

Referral Form

All service providers, including police and solicitors, who are referring a defendant to Life on Track, must complete the Life on Track Referral Form. Once completed, the form can be faxed, emailed, mailed or delivered to the local Life on Track service provider.
